What do I need to bring to an eye examination?

Please bring the following to your eye examination where applicable:

  • OHIP card
  • insurance information and forms
  • medication list
  • current eye glasses
  • contact lenses and case
  • contact lens information such the original box (if not already on file)
  • sunglasses 

Some people will prefer to bring a driver since pupil dilation is often performed.
